朱永安意见调查表格

Hi Nantah Alumni,

Do Nantah alumni care about Nantah and what are their opinions? This is a confidential poll to survey alumni's opinions on 3 current Nantah issues. You may answer some or all three questions and you may choose not to give reasons. Please write "(1) yes/no, (2) yes/no, (3) yes/no" and include your brief reasons (optional). Please e-mail your answers to me (enguchoo@yahoo.ca).

I will not post your viewpoint by individual name. I will post the survey results in four parts: (Part 1) list of names of alumni who participated in the survey, (Part 2) number of "yes" and "no" for each of the three questions, (Part 3) list of reasons given for each "yes", (Part 4) list of reasons given for each "no".

You are welcome to collect survey answers from other alumni in your groups and prepare the four part survey results. I will combine all survey results with care to avoid double counting any names.

Survey is a convenient and easy way for more of us to show our viewpoints. Please do express your viewpoints so that some unified Nantah initiatives may be discovered.
